The Chandler Health Community
Chandler Health is a new primary care practice established by Sarah Chandler DNP, AGNP-C. Sarah has been providing quality health care for over 20 years and is focused on helping patients feel informed, supported, and empowered in their health care journey. Sarah will be moving her clinic patients from Stark Medical, and her house call patients from Ancora over to Chandler Health.
The goal of this transition is to continue providing patients with the resources and quality care they need, all under one practice. At Chandler Health we know this can be a confusing time.
